Honestly, I don't have a copy of such a letter mostly because the letter will largely depend on your personal situation ie. how long were you with the company, type of job, your reputation with the company/your manager, etc.

In that regard, the content of the letter would largely depend on your individual personal situation.

Having said that, I'd think that if you already worked for the company and had a good reputation, that you'd be better off simply making your intentions known directly ie. pick up the phone and speak with someone in the company who could potentially hire you, as opposed to sending them a cover letter and resume. In this case, since they know you, you might be able to bypass the "traditional" cover letter and resume application process.
