When resigning due to illness, it's always good to think of it this way: even if you have no intention of coming back to work with this particular company, it's good to keep all doors open, not burn any bridges and leave on good terms.

Dear Jane

Please accept this letter as confirmation of my resignation from my position as Your Job Title Here with ABC Corporation.

As you are aware, I have been ill for an extended period of time and no longer feel that I can continue working with ABC Corporation. I feel it's in the best interest of both the company and myself to resign and have done so with a great deal of thought put into the decision.

I wish to leave on good terms and will do my best to finish up my work and to assist with a smooth transition before leaving.
